This form is if you need to update a QualCert’d Driver who is already in Impact’s Safety Trainer Hub, whether due to incorrectly entered information and/or new/changed information (i.e. adding a Road Test or Observation Ride; updating an email address, etc.).

I will give the driver a copy of the road test and if the vehicle is above 10K lbs upload to FADV. Additionally, I will give a copy of the observed hours form to station personnel if required.
