This form is if you are submitting/onboarding a new driver you trained for Qualification Certification.
The QualCert Reminders below are designed to help guide you through the required steps of the program. Please check each item off as an indication they’ve been completed. There are three stages that need to be completed consecutively:
Stage 1 – These need to be completed and submitted prior to completing the Road Test.
Stage 2 – The Road Test needs to be uploaded as indicated prior to the commencement of Stage 3. (*Upload under “Update Driver” tab*)
Stage 3 – The Observation Ride. (*Upload under “Update Driver” tab*)
REMINDER: After submitting your signature, use the “Update Driver” tab to upload the Road Test (if applicable) and the Observation Ride. Also, don’t forget to give your driver and FADV a copy of their certificate.
