This form is if you are submitting a RECERT driver that IS moving up a training level (i.e. this driver was QualCertd on a P700 but is now going to operate a larger CMV).

The QualCert Reminders below are designed to help guide you through the required steps of the program. Please check each item off as an indication they’ve been completed. There are
three stages that need to be completed consecutively:
Stage 1 – These need to be completed and submitted prior to
completing the Road Test.
Stage 2 –  The Road Test needs to be uploaded as indicated
prior to the commencement of Stage 3. (*Upload under “Update
